Admission $25 per person in advance / $30 per person at the door
Contributions are tax-deductible. All ages welcome. Sorry, group discounts not available.

1) Fill out registration form below
2) Make checks payable to First Unitarian Church; Write “Swing Dance” on the memo line of the check
3) Enclose a list of who you are sending money for
4) Enclose a phone number so we can contact you if there are questions
5) Write the check for $25 for each person attending
6) Mail check to: Swing Dance First Unitarian Church 569 S. 1300 East Salt Lake City, UT 84102

2) We must have the contact info requested (below) in order to register you for the swing dance
3) Even if you are already registered with the church, we need you to fill out this form for the dance
3) After filling out the registration form and hitting the "Submit" button, click the "Donate" button on the bottom of this page
4) The "Donate" button will take you to a new page. Do not click the "Donate" button until you have filled out the registration form.
5) On the "Donate" form, fill in your amount next to "Swing Dance Fundraiser. "
